Priority Service Restoration

If you or someone in your household has a qualifying medical condition, you may be eligible for priority natural gas service restoration if there is an interruption in your service due to an outage. Select the link for your area to review a Certification of Health and/or Disability Form.
 

  • Print the Certification of Health and/or Disability form
  • Have a licensed physician, public health official or social worker complete the form
  • Sign and return it to Southwest Gas
